Medication that is to be administered to your child in school must be accompanied with a signed doctor's note giving instructions as well as a signed note from the parents.
 For health reasons, gum is not allowed on the school grounds. Food, candy (discouraged) or Chinese preserve seeds are to be eaten only during lunch periods. No unshelled sunflower seeds.

MEDICAL REQUIREMENTS

Physical examinations are required for all children entering school for the first time in Hawaii. All new students will be required to provide the school with a completed DOE Form 14 Health Record by a family physician which documents:

a. Tuberculin test (Mantoux type) with negative results or follow up x-ray. (TB test must be given not more than 12 months before school entrance.)
b. Complete or current immunizations
c. Physical examination

Returning students in grades 4 and 7 must provide documentation showing evidence of physical examination, including scoliosis check for 7th grade students.
